Documentation for an RN moving into a practitioner role can be daunting especially for me with 30+ years of charting in a nurse role. This book is a nice reference for a variety of clinical areas; acute care and primary care. I am glad I found it and am happy with the examples provided as I move into thinking and charting as a mid-level provider.

Great help for SOAP NOTES.
As a 3rd year Medical Student, I purchased this book for use while in the Wards. I was given two weeks notice before heading to the Wards and practiced the material in the book at least once. The shorthand for both consultations and progress notes, as well as prescription shorthand, was very useful. I was praised by my Attending Physicians on the quality and expediency of my notes and I give all credit to this book. I highly recommend it to fellow Medical Students as well as anyone that comments on my clinical documents.
It has been a great resource for my first clinical rotation. I've recommended this book to several NP students and suggested it to my professor for the upcoming classes. There is so much valuable information that I can use it for effective charting in several situations.
